Wat is een ijzertekort?

Wat is de link tussen ijzer en transferrine, ijzer en hemoglobine of ijzer en ferritine tekort?

Wat wordt er getest?
Bij dit onderzoek worden verschillende bloed- en speekselwaarden gemeten die belangrijke informatie geven over onder andere zuurstoftransport, stressniveau, schildklierfunctie, ijzerstatus en nierwerking. Hieronder vind je een overzicht van alle markers en wat ze betekenen.
Hematocriet
De hematocrietwaarde geeft aan welk deel van het bloedvolume bestaat uit rode bloedcellen. Wanneer de hematocriet bijvoorbeeld 40% is, bestaat 100 milliliter bloed uit 40 milliliter rode bloedcellen. Deze waarde zegt veel over het vermogen van het bloed om zuurstof te vervoeren; afwijkingen kunnen wijzen op bloedarmoede of juist op een teveel aan rode bloedcellen.
Hemoglobine (Hb)
Hemoglobine is een eiwit in rode bloedcellen dat zuurstof bindt en vervoert. Elk hemoglobinemolecuul bevat vier ijzeratomen, die ieder één zuurstofmolecuul kunnen vasthouden. Cortisol (speeksel)

TSH
TSH (Thyreoïdstimulerend Hormoon) wordt gemaakt door de hypofyse en stuurt de schildklier aan om T3 en T4 te produceren. Via een terugkoppelingsmechanisme wordt TSH gereguleerd:

fT4
fT4 (vrij thyroxine) is de niet-actieve vorm van schildklierhormoon die vrij in het bloed circuleert. Het grootste deel is aan eiwitten gebonden. In de lever wordt fT4 omgezet in het actieve T3. Samen met TSH geeft fT4 een goed beeld van de schildklierfunctie.
Creatinine
Creatinine ontstaat bij de afbraak van creatinefosfaat in de spieren en wordt via de nieren uitgescheiden. Het is een belangrijke marker om te beoordelen hoe goed de nieren werken: wanneer de nierfunctie vermindert, stijgt het creatininegehalte in het bloed.
eGFR
De estimated Glomerular Filtration Rate (eGFR) schat hoeveel bloed de nieren per minuut filteren. Waarden boven 90 ml/min wijzen op een gezonde nierfunctie. Een eGFR onder 15 ml/min duidt op eindstadium nierfalen, waarbij dialyse vaak noodzakelijk is. Geslacht en leeftijd spelen een grote rol bij de interpretatie.
Transferrine
Transferrine transporteert ijzer door het bloed. Bij ijzertekort stijgt de transferrinewaarde omdat het lichaam meer transporteiwitten aanmaakt. Een lage waarde kan wijzen op ijzerstapeling, leverproblemen of eiwittekort. Zwangerschap en de anticonceptiepil kunnen de waarde verhogen.
Ijzer (serumijzer)
IJzer is essentieel voor de aanmaak van hemoglobine. Een ijzertekort kan leiden tot bloedarmoede (anemie). De ijzerstatus wordt het best beoordeeld in combinatie met ferritine, transferrine en transferrinesaturatie.
