De nieren
De nieren zijn twee organen die aan de rugzijde, ter hoogte van de onderste ribben gelegen zijn. De nieren hebben een aantal heel belangrijke taken, waaronder het reguleren van de water- en zouthuishouding; het behouden van een gezonde bloeddruk; het stimuleren van de aanmaak van rode bloedcellen; het behouden van de juiste zuurtegraad in het bloed en natuurlijk het filteren van het bloed om het te ontdoen van gifstoffen. Maar liefst 25% van het totale bloedvolume stroomt per minuut door de nieren!
Om aan die belangrijke taak te kunnen voldoen, hebben de nieren een enorme capaciteit om het bloed te filteren.
Wat is nierfunctie?
De nierfunctie zelftest meet de hoeveelheid creatinine in het bloed. Creatinine is een afbraakproduct afkomstig van de spieren. Het wordt continu door de nieren uit het bloed gefilterd en uitgescheiden via de urine. Het is daarom een goede maat voor de nierfunctie. Is de functie van de nieren verminderd, dan zullen de nieren minder goed in staat zijn om creatinine uit het bloed te filteren. Creatinine zal achterblijven in het bloed en de waarde zal stijgen.
Via een formule, waarin het creatinine wordt meegenomen, wordt de nierfunctie geschat. Deze schatting heet eGFR, ofwel estimated Glomerular Filtration Rate. De nierfunctie is sterk afhankelijk van leeftijd en geslacht.
De nierfunctie is in feite de hoeveelheid bloed die per tijdseenheid door de nierfilters kunnen passeren. Het exact meten van de nierfunctie is omslachtig en tijdrovend. Daarom wordt in de praktijk de nierfunctie vaker geschat met behulp van de stof creatinine. Creatinine is een afbraakproduct afkomstig van spieren. Wat is een normale nierfunctie?
Gezonde nieren van jonge mensen zouden meer dan 90 milliliter bloed per minuut moeten kunnen filteren. Ouderen kunnen lagere waardes hebben. Het wordt gevaarlijk wanneer de nierfunctie minder dan 15 milliliter bloed per minuut bedraagt.
Verminderde nierfunctie symptomen
Een licht verminderde nierfunctie geeft geen symptomen. Vaak ervaren patiënten pas symptomen wanneer er al ernstig nierfalen is.
De symptomen kunnen zijn:
Hoge bloeddruk en bijbehorende klachten, omdat de nieren de balans tussen hoeveelheid vocht en zout in het lichaam regelen.
Bloedarmoede, omdat de nieren EPO maken, nodig voor de aanmaak van rode bloedcellen.
Vermoeidheid als gevolg van opstapeling van afvalstoffen.
Spierkrampen als gevolg van disbalans in elektrolyten zoals magnesium, natrium en kalium.
Jeuk en huidproblemen als gevolg van opstapeling van afvalstoffen.
Schuimende urine, omdat er eiwitten in de urine terecht komen.
De zelftest voor sporters richt zich specifiek op mensen die intensief sporten. Met de eenvoudige bloedtest voor sporters krijg je via een vingerprik inzicht in je bloedwaarden. Wat wordt er getest?
Bij dit onderzoek worden verschillende bloed- en speekselwaarden gemeten die belangrijke informatie geven over onder andere zuurstoftransport, stressniveau, schildklierfunctie, ijzerstatus en nierwerking. Hieronder vind je een overzicht van alle markers en wat ze betekenen.
Hematocriet
De hematocrietwaarde geeft aan welk deel van het bloedvolume bestaat uit rode bloedcellen. Wanneer de hematocriet bijvoorbeeld 40% is, bestaat 100 milliliter bloed uit 40 milliliter rode bloedcellen. Deze waarde zegt veel over het vermogen van het bloed om zuurstof te vervoeren; afwijkingen kunnen wijzen op bloedarmoede of juist op een teveel aan rode bloedcellen.
Hemoglobine (Hb)
Hemoglobine is een eiwit in rode bloedcellen dat zuurstof bindt en vervoert. Elk hemoglobinemolecuul bevat vier ijzeratomen, die ieder één zuurstofmolecuul kunnen vasthouden. Een te lage Hb-waarde kan duiden op bloedarmoede, terwijl een te hoge waarde onder andere kan voorkomen bij rokers of mensen die op grote hoogte leven.
Cortisol (speeksel)
Cortisol staat bekend als het stresshormoon en wordt geproduceerd door de bijnieren. Het helpt het lichaam omgaan met stress door processen zoals glucoseproductie te stimuleren. Cortisol volgt een dag-nachtritme: 's ochtends hoog, ’s nachts laag. Daarom moet speeksel voor deze test ’s ochtends worden afgenomen. Afwijkingen kunnen wijzen op overmatige stress of aandoeningen zoals de ziekte van Cushing (te veel cortisol) of Addison (te weinig cortisol). Let op: de anticonceptiepil kan cortisolwaarden verhogen.
TSH
TSH (Thyreoïdstimulerend Hormoon) wordt gemaakt door de hypofyse en stuurt de schildklier aan om T3 en T4 te produceren. Via een terugkoppelingsmechanisme wordt TSH gereguleerd:
Te hoge TSH wijst meestal op een traag werkende schildklier.
Te lage TSH past bij een snel werkende schildklier.
fT4
fT4 (vrij thyroxine) is de niet-actieve vorm van schildklierhormoon die vrij in het bloed circuleert. Het grootste deel is aan eiwitten gebonden. In de lever wordt fT4 omgezet in het actieve T3. Samen met TSH geeft fT4 een goed beeld van de schildklierfunctie.
Creatinine
Creatinine ontstaat bij de afbraak van creatinefosfaat in de spieren en wordt via de nieren uitgescheiden. Het is een belangrijke marker om te beoordelen hoe goed de nieren werken: wanneer de nierfunctie vermindert, stijgt het creatininegehalte in het bloed.
eGFR
De estimated Glomerular Filtration Rate (eGFR) schat hoeveel bloed de nieren per minuut filteren. Waarden boven 90 ml/min wijzen op een gezonde nierfunctie. Een eGFR onder 15 ml/min duidt op eindstadium nierfalen, waarbij dialyse vaak noodzakelijk is. Geslacht en leeftijd spelen een grote rol bij de interpretatie.
Transferrine
Transferrine transporteert ijzer door het bloed. Bij ijzertekort stijgt de transferrinewaarde omdat het lichaam meer transporteiwitten aanmaakt. Een lage waarde kan wijzen op ijzerstapeling, leverproblemen of eiwittekort. Zwangerschap en de anticonceptiepil kunnen de waarde verhogen.
Ijzer (serumijzer)
IJzer is essentieel voor de aanmaak van hemoglobine. Een ijzertekort kan leiden tot bloedarmoede (anemie). De ijzerstatus wordt het best beoordeeld in combinatie met ferritine, transferrine en transferrinesaturatie. Een lage ijzerwaarde samen met een lage ferritinewaarde is een sterke aanwijzing voor een ijzertekort.
Transferrinesaturatie
Deze waarde wordt berekend uit de hoeveelheid ijzer en transferrine in het bloed. Ze geeft aan welk percentage van het transferrine verzadigd is met ijzer. Een lage transferrinesaturatie duidt vaak op een ijzertekort.
Ferritine
Ferritine slaat ijzer op in de lever. Wanneer het lichaam extra ijzer nodig heeft, wordt deze voorraad aangesproken en daalt de ferritinewaarde. Een lage ferritine in combinatie met een laag serumijzer is typisch voor een ijzertekort.
Ben je vaak moe? Heb je weinig eetlust of voel je je mentaal minder scherp? Dan heb je misschien last van een te laag ijzergehalte. Veel mensen hebben namelijk een ijzertekort zonder dat ze het weten. Je eventuele ijzertekort meten doe je eenvoudig met een Easly test.
Met een zelftest van Easly meet je thuis het ijzer in bloed dat je zelf hebt afgenomen. Met een vingerprik neem je ’s ochtends bloed voor je eerste maaltijd. Het staal stuur je naar ons lab waar het grondig wordt geanalyseerd. Ongeveer 48 uur na ontvangst op het laboratorium kan je de resultaten inzien.
Wat is ijzer?
IJzer is een belangrijk mineraal. Ons lichaam maakt ijzer niet zelf, maar haalt het uit onze voeding. Het is nodig om hemoglobine aan te maken. Hemoglobine zorgt ervoor dat zuurstof door het lichaam kan worden getransporteerd.
Wat is een ijzertekort?
Een ijzertekort uit zich meestal in bloedarmoede, ook wel anemie genoemd in medisch vakjargon. Een tekort aan ijzer in bloed leidt namelijk tot een tekort aan hemoglobine en dat staat gelijk aan bloedarmoede.
Een laag resultaat op een ijzer test kan verschillende oorzaken hebben:
een gebrek aan ijzerrijke voeding.
een verminderd vermogen om ijzer uit de voeding op te nemen.
een verlies van ijzer.
De laatste categorie, ‘verlies van ijzer’, is de meest voorkomende oorzaak van een ijzertekort. Verlies van ijzer gebeurt meestal door chronisch bloedverlies. Dat kan onschuldig zijn, als gevolg van hevige menstruaties bij vrouwen in de vruchtbare fase; maar ook minder onschuldig, bijvoorbeeld als gevolg van bloedverlies uit het maagdarmstelsel door een tumor. Daarnaast zijn er ook nog andere oorzaken voor verlies van ijzer, bijvoorbeeld als gevolg van een chronische ontsteking, een auto-immuunziekte of een maligniteit.
Wat is de link tussen ijzer en transferrine, ijzer en hemoglobine of ijzer en ferritine tekort?
IJzer in ‘vrije vorm’ komt in het lichaam maar in zeer lage concentraties voor. In het bloed is ijzer grotendeels gebonden aan een transporteiwit, genaamd transferrine.
IJzer dat niet meteen gebruikt wordt, wordt opgeslagen in de lever dankzij het bindingseiwit ferritine. Tekort aan ferritine en teveel aan transferine zijn dus ook interessante waarden om te meten bij een ijzer tekort test.
Een laag ijzergehalte kan leiden tot een tekort aan hemoglobine, oftewel bloedarmoede.
Hoe is een tekort aan ijzer in bloed waarden te zien?
Met een Easly ijzer test kan je heel eenvoudig ieders ijzergehalte (vrouw/man) meten.
Vrouwen in de vruchtbare fase van het leven hebben een hoger ijzergehalte nodig dan mannen, omdat ze via de menstruatie meer ijzer verliezen. Ook zwangere vrouwen hebben meer ijzer nodig, noodzakelijk voor de aanmaak van extra bloed tijdens de zwangerschap. Indien het ijzergehalte in het begin van de zwangerschap sterk afwijkt van de ijzer normaalwaarde kan een ijzersupplement nodig zijn, aangezien de ijzervoorraad via de voeding slechts zeer traag wordt aangevuld.
Wat zijn de symptomen van ijzertekort?
De meest voorkomende klachten bij een te lage ijzer waarde zijn:
Vermoeidheid
Spierzwakte
Een bleke huid
Wondjes in de mondhoeken (ragaden)
Brokkelige nagels met groeven
Een droge huid
Haaruitval
Een branderig gevoel en roodheid van tongslijmvlies.
Waarom je ijzergehalte testen en wanneer?
